The Planning Board office is now a Passport Application location. Pick up application forms here. Passport photos available at CVS and AAA locations

Basic Requirements for Applying for the first time, renewing a minor's passport or if your passport was issued more than 15 years ago:

• Raised seal birth certificate with parent's names - NOT a Hospital Certificate
• Photo ID - Current Driver's License, military ID, etc.
• 2 photos (2"x2") - availabe at AAA, CVS
• Previously issued passport (if issued more than 15 years ago and expired)
• 1 check for $25 to the Town of Bourne (per application)
• 1 check per application for $75 (age 16 and older) or $60 (if under 16) to the US Dept. of State 
• Expidited Service - guaranteed 3 weeks. Add $60 and $16.25 for overnight delivery.
• Applying for a minor (under 16) - both parents need to be present or notarized consent from one parent or proof of sole custody.

Requirements for renewing a passport

• You can submit most recent passport
• You were at least 16 when most recent passport was issued
• You were issued your most recent passport less than 15 years ago
• You use the same name as on the most recent passport
  • Or you have had your name changed by marriage or court order and can submit proper documentation to reflet name change
• Fee: $75 per application made to the U.S. Dept. of State. - date of birth must be on check
• 2 new photos must be submitted
• Current passport must be submitted

Fees must be in check or money order form
