For a copy of a fire incident report for a response to your own property please contact the business office at 508-759-0600.

For Ambulance Billing Inquiries:

For 21E/Oil or Hazardous Materials Release Requests:

For 21E requests, fill out the appropriate application here. There is a $50.00 processing fee that can be paid for online.
