Aflac and Cafeteria Plan Enrollment

This page contains information about Aflac and Cafeteria Plan Advisors open enrollment.  These benefits are available to full time, permanent employees.

Cafeteria Plan Advisor benefits are entirely voluntary and paid 100% by the employee.  Enrollment for the Health Care Flexible Spending Account and the Dependent Care Flexible Spending Account can be combined or done separately.  Enrollment forms need to be returned to the Cafeteria Plan Advisors, Inc by email or fax or enrollment can be completed online.  Please see enrollment form for information.  You must re-enroll EVERY YEAR.  The deadline to complete enrollment is 11/30/2021.
